Important Insurance Information

Insurance Coverage for Athletes

Please be advised of the procedures to be followed in the case of an injury to a student athlete:

• Supervising coach fills out and injury report and submits to the school nurse where the team is located.
• The athlete reports to the nurse on the next day to fill out an injury report.
• If the parents have insurance (noted on the emergency authorization card), they must take the child to a participating provider for their insurance.
• School insurance picks up any co-pay or in excess charges.
• All bills must be submitted to family insurance company first.
• No bills are to be submitted to the school nurse.
